Enjoy the autonomy of per diem work by providing essential care in a home health setting. This Nurse Practitioner opportunity involves conducting comprehensive home health risk assessments for Medicare Advantage plan members across various communities in Fayette County, Ohio. Engage with members in their residences, offering impactful support through educational discussions and clinical evaluationswithout the need for ongoing treatment or prescription duties.
Schedule:
- Flexible shifts available any day between Monday and Sunday
- Self-select and manage your schedule
- Commitment required for a minimum number of hours weekly or bi-weekly
What Youll Do:
- Perform thorough annual or post-discharge home health risk assessments, averaging 45 minutes each
- Review and document current and past medical histories, medical conditions, and medication usage
- Identify gaps in care to support early interventions and improve outcomes
- Educate patients on their health conditions and proper medication management
- No prescriptive or treatment plan management involved
Desired Qualifications:
- Active Nurse Practitioner license in Ohio
- Current CPR (BLS) certification
- Board Certified through AANP or ANCC
- MSN degree from an accredited nursing program
- At least 6 months of recent nurse practitioner experience preferred
This per diem role offers compensation of $80-$90 per evaluation, with additional mileage and administrative time stipends.
