Deliver comprehensive patient care and support in a dynamic per diem nurse practitioner position focused on home health risk assessments for Medicare Advantage plan members. This role offers flexible scheduling and the opportunity to make a significant impact throughout communities in and around Jamestown as well as Chautauqua County. Work independently while visiting members in their homes, ensuring personalized and high-quality care with each assessment.

Schedule:

• Flexible shifts available 7 days a week
• Freedom to create your own work schedule
• Set minimum weekly or bi-weekly commitment to fit your availability

Key Responsibilities:

• Perform annual and post-discharge risk assessments in members homes
• Assess prior and current medical history, conditions, and medication use
• Provide education related to medical conditions, preventive care, and medication management
• Identify gaps in care and recommend solutions to optimize patient outcomes
• Visits average approximately 45 minutes; duties do not include treatment plan creation or medication prescribing

Qualifications:

• Active Nurse Practitioner license in NY State
• Current CPR (BLS) certification
• Board Certification as a Nurse Practitioner through AANP or ANCC
• MSN from an accredited school of nursing
• At least 6 months of NP experience preferred
