Job Description

Experienced Nurse Practitioners are invited to provide comprehensive home health risk assessments for Medicare Advantage plan members residing throughout New York, NY. This per diem opportunity allows for maximum flexibility, as practitioners create their own schedules and shifts are available seven days a week. Expect a minimum weekly or bi-weekly hourly commitment, with the ability to conduct visits in various New York locations during a shift.

Schedule:

  • Flexible shifts available Monday through Sunday

  • Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

  • Conduct in-depth annual health risk assessments for Medicare Advantage members in their homes
  • Review and evaluate patients current and previous medical history, conditions, and medications
  • Educate patients on their current health conditions and prescribed medications
  • Identify care gaps, address patient needs, and recommend actions for improved care continuity
  • Visits may include annual health assessments or post-discharge evaluations
  • No medication prescribing or ongoing treatment plans required

Qualifications:

  • Active Nurse Practitioner license in New York
  • Master of Science in Nursing (MSN) from an accredited institution
  • Board Certification through AANP or ANCC
  • Current CPR certification (BLS)
  • At least six months of experience as a Nurse Practitioner preferred
  • Strong assessment, communication, and patient education skills

This position offers competitive compensation$80-$90 per completed evaluationalong with a mileage stipend and additional pay for administrative time.
