This per diem opportunity offers exceptional flexibility with self-scheduling between Monday and Sunday, enabling you to create your own work-life balance based on your availability and preferences. You will perform comprehensive risk assessments with Medicare Advantage patients in their homes, supporting their understanding of health conditions while identifying critical care needs.
Schedule:

Key Responsibilities:
- Conduct annual home health risk assessments for Medicare Advantage plan members
- Obtain detailed medical history, review current and prior conditions, and assess medication regimens
- Educate patients regarding diagnoses, medications, and care principles
- Recognize gaps in healthcare and provide actionable recommendations
- No treatment planning or prescribing requiredfocus is assessment and education
Qualifications:
- Active Nurse Practitioner license in New York
- Current CPR certification (BLS)
- National Board Certification (AANP or ANCC)
- Minimum of 6 months experience as an NP preferred
- MSN degree from an accredited program
