You will perform comprehensive risk assessments with Medicare Advantage patients in their homes, supporting their understanding of health conditions while identifying critical care needs.

Schedule:

  • Minimum weekly or bi-weekly hourly commitment required

Key Responsibilities:

  • Conduct annual home health risk assessments for Medicare Advantage plan members
  • Obtain detailed medical history, review current and prior conditions, and assess medication regimens
  • Educate patients regarding diagnoses, medications, and care principles
  • Recognize gaps in healthcare and provide actionable recommendations
  • No treatment planning or prescribing requiredfocus is assessment and education

Qualifications:

  • Active Nurse Practitioner license in New York
  • Current CPR certification (BLS)
  • National Board Certification (AANP or ANCC)
  • Minimum of 6 months experience as an NP preferred
  • MSN degree from an accredited program
