Job Description
Seeking a dedicated Nurse Practitioner to provide home health risk assessments for Medicare Advantage plan members. In this per diem role, you will visit patients in their homes to perform comprehensive evaluations, focusing on annual health assessments and post-discharge appointments. Each assessment typically lasts about 45 minutes and is essential to ensuring optimal patient outcomes and care continuity. Flexibility is key: your schedule is self-directed, with shifts available any day of the week.
Schedule:
- Flexible shifts between Monday and Sunday
- Set your own workdays and hours
- Minimum weekly or bi-weekly hourly commitment
Responsibilities:
- Conduct in-home health risk assessments for Medicare Advantage members
- Gather and evaluate medical histories, current conditions, and medication usage
- Educate patients regarding their health conditions and prescribed medications
- Identify gaps in care and propose actionable recommendations for improvement
- Focus solely on assessmentno requirement for treatment planning or prescribing
Qualifications:
- Active Nurse Practitioner license for New York State
- MSN degree from an accredited school of nursing
- Current BLS (CPR) certification
- Board certification through AANP or ANCC
- Minimum of 6 months of nurse practitioner experience preferred
This opportunity offers compensation of $80-$90 per evaluation, with additional mileage reimbursement and administrative time provided.
