Step into a vital, patient-centered role as a Nurse Practitioner specializing in home health risk assessments. This per diem opportunity offers flexibility and autonomy, allowing you to manage your own schedule while making a meaningful impact on Medicare Advantage plan members residing in the Bronx, NY. As a valued healthcare professional, youll be responsible for conducting comprehensive evaluations in the comfort of patients homes, with each assessment typically lasting around 45 minutes. The focus of each visit may range from annual health assessments to post-discharge evaluations, ensuring continuity and quality of care.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Set your own schedule for maximum work-life balance
  • Commit to a minimum number of weekly or bi-weekly hours

Responsibilities:

  • Perform annual home health risk assessments for Medicare Advantage plan members
  • Review and evaluate prior and current medical history, medical conditions, and medication usage
  • Deliver patient education regarding health conditions and medications
  • Identify care gaps and make clinical recommendations to close those gaps
  • No requirement to create treatment plans or prescribe medications

Qualifications:

  • Active Nurse Practitioner license in NY
  • Board Certification via AANP or ANCC
  • Current BLS certification (CPR)
  • MSN degree from an accredited institution
  • A minimum of six months recent experience as an NP is preferred

Your commitment and expertise will be recognized through compensation of $80$90 per evaluation, with additional mileage stipend and reimbursement for administrative time.
