Enjoy the flexibility of per diem work as you deliver critical care for Medicare Advantage plan members throughout Onslow County, NC. In this role, youll provide comprehensive home health risk assessments in communities including Jacksonville, Holly Ridge, and Swansboro, helping to ensure continuity of care and preventive support for patients in their homes.

Schedule:

• Flexible shifts between Monday and Sunday
• Create your own schedule
• Minimum weekly or bi-weekly hourly commitment

Responsibilities:

• Perform annual home health risk assessments for Medicare Advantage members
• Assess patients prior and current medical history, existing conditions, and medication usage
• Teach patients about their health conditions and prescribed medications
• Identify gaps in care and make recommendations to improve patient outcomes
• No treatment plan creation or medication prescribing required

Qualifications:

• Active Nurse Practitioner license in North Carolina
• Current CPR certification (BLS)
• Board certification through AANP or ANCC
• At least 6 months of recent Nurse Practitioner experience preferred
• MSN degree from an accredited nursing program
