Take the next step in your Nurse Practitioner career by delivering high-quality, patient-centered home health assessments. This per diem opportunity involves conducting risk assessments for Medicare Advantage plan members in Jacksonville, Holly Ridge, Swansboro, and surrounding areas within Onslow County, NC. Youll play a crucial role in evaluating patients health in the comfort of their homessupporting continuity of care without treatment planning or prescribing.
Schedule:
- Flexible shifts available Monday through Sunday
- Self-schedulingchoose your own hours
- Minimum commitment required weekly or bi-weekly
Key Responsibilities:
- Complete annual home health risk assessments for Medicare Advantage participants
- Review and evaluate patients medical history, current conditions, and medication use
- Offer education to patients regarding their health conditions and medication regimens
- Identify gaps in care and suggest actionable recommendations
- Documentation and reporting aligned with established protocols
- No responsibility for prescribing medications or creating/following treatment plans
Qualifications:
- Active Nurse Practitioner license in North Carolina
- CPR (BLS) certification
- Board certified through AANP or ANCC
- Completion of an MSN from an accredited program
- At least 6 months of current NP experience preferred
