Job Description

This per diem opportunity involves conducting risk assessments for Medicare Advantage plan members in Jacksonville, Holly Ridge, Swansboro, and surrounding areas within Onslow County, NC. Youll play a crucial role in evaluating patients health in the comfort of their homessupporting continuity of care without treatment planning or prescribing.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Self-schedulingchoose your own hours
  • Minimum commitment required weekly or bi-weekly

Key Responsibilities:

  • Complete annual home health risk assessments for Medicare Advantage participants
  • Review and evaluate patients medical history, current conditions, and medication use
  • Offer education to patients regarding their health conditions and medication regimens
  • Identify gaps in care and suggest actionable recommendations
  • Documentation and reporting aligned with established protocols
  • No responsibility for prescribing medications or creating/following treatment plans

Qualifications:

  • Active Nurse Practitioner license in North Carolina
  • CPR (BLS) certification
  • Board certified through AANP or ANCC
  • Completion of an MSN from an accredited program
  • At least 6 months of current NP experience preferred
