Job Description

Experienced Nurse Practitioners are invited to join a dynamic home health role, focusing on comprehensive in-home risk assessments for Medicare Advantage plan members. This per diem opportunity offers unmatched scheduling flexibility and the autonomy to serve patients across Jacksonville and surrounding areas within Onslow County.

Schedule

  • Flexible shifts available Monday through Sunday
  • Self-scheduling to fit your preferred availability
  • Options for minimum weekly or bi-weekly hourly commitments

Key Responsibilities

  • Complete thorough annual home health risk assessments for Medicare Advantage health plan members
  • Review and evaluate patients medical history, current conditions, and medication usage
  • Educate members about their health concerns, medications, and the importance of ongoing care
  • Identify any care gaps and provide recommendations for follow-up, supporting improved patient outcomes
  • Focus strictly on assessmentno prescribing or development of treatment plans necessary

Qualifications

  • Active Nurse Practitioner License in North Carolina
  • Up-to-date CPR certification (BLS required)
  • Board Certified through AANP or ANCC
  • At least 6 months of recent experience as an NP (preferred)
  • Masters degree in Nursing (MSN) from an accredited program
