Step into a dynamic per diem opportunity tailored for experienced Nurse Practitioners with a passion for home health. Deliver impactful patient care as you conduct home health risk assessments for Medicare Advantage members throughout Duval County. This role allows you to work directly within the community, with the flexibility to choose your schedule and locationsincluding areas such as Jacksonville, Jacksonville Beach, and Atlantic Beach.

- Choose your weekly or bi-weekly hourly commitment
Key Responsibilities
- Conduct comprehensive home health risk assessments on Medicare Advantage plan members
- Review prior and current medical history as well as medication history
- Educate patients on health conditions and medications during visits
- Identify gaps in care and provide actionable recommendations
- No prescribing or creation of treatment plans required; focus is on evaluation and education
- Each assessment averages 45 minutes
Qualifications
- Active Nurse Practitioner License (FL)
- Current certification in CPR/BLS
- Board certified through the AANP or ANCC
- At least 6 months of recent NP experience preferred
- MSN degree earned from an accredited nursing program
You'll earn between $80 and $90 per evaluation, in addition to a mileage stipend and compensation for necessary administrative time related to your visits.
