Take advantage of a flexible per diem opportunity to deliver personalized healthcare in the community setting. This position offers Nurse Practitioners the chance to conduct comprehensive home health risk assessments for Medicare Advantage members throughout Duval County, including Jacksonville, Jacksonville Beach, and Atlantic Beach. Each evaluation takes approximately 45 minutes and may include annual health checkups or post-discharge visits.
Schedule:
- Flexible shifts available Monday through Sunday
- Design your own schedule
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Perform annual home health risk assessments for Medicare Advantage plan members in their homes
- Review and evaluate medical history, existing medical conditions, and medication regimens
- Educate patients about their health conditions and proper medication usage
- Identify gaps in care and recommend resolutions to improve patient outcomes
- Note: This role does not involve developing treatment plans or prescribing medications
Qualifications:
- Active Nurse Practitioner license in Florida
- Board Certified Nurse Practitioner (AANP or ANCC)
- Current CPR certification (BLS)
- Graduation from an accredited MSN program
- At least six months of current NP experience preferred
Nurse Practitioners in this role typically earn $80$90 per evaluation, with additional mileage stipend and compensation for administrative time.
