Job Description

This per diem opportunity is ideal for an experienced Nurse Practitioner interested in conducting home health risk assessments for Medicare Advantage plan members throughout Madison County. The role offers substantial flexibility, allowing you to create your own schedule and select shifts between Monday and Sunday, with a minimum weekly or bi-weekly hourly commitment. Assignments may include visits in local areas such as Jackson, Medon, or Three Way.

Key Responsibilities:

  • Perform annual home health risk assessments for Medicare Advantage plan members
  • Thoroughly review medical histories, conditions, and current medication lists
  • Provide education to members regarding their health conditions and medication regimens
  • Identify and document quality or care gaps, recommending resolution strategies
  • No requirement to develop treatment plans or prescribe medications

Qualifications:

  • Current Nurse Practitioner License in Tennessee
  • Board Certification through AANP or ANCC
  • CPR certification (BLS)
  • Minimum of 6 months recent experience as an NP preferred
  • Master of Science in Nursing (MSN) from an accredited institution
