This per diem opportunity involves conducting quality home health risk assessments for Medicare Advantage members throughout Gibson County, Tennesseeincluding Humboldt, Milan, and Trenton. Schedule:

• Flexible shift options available Monday through Sunday
• Self-scheduling to support work-life balance
• Minimum weekly or bi-weekly hour commitment

Key Responsibilities:

• Perform annual risk assessments and post-discharge visits, each typically lasting around 45 minutes
• Review and evaluate members medical and medication histories
• Deliver education on existing medical conditions and current medications
• Identify potential gaps in care and provide constructive recommendations
• No treatment planning or medication prescribing duties

Qualifications:

• Active Nurse Practitioner license in Tennessee
• Board Certification via AANP or ANCC required
• Master of Science in Nursing (MSN) from an accredited institution
• Valid CPR (BLS) certification
• At least six months of recent NP experience preferred
