Deliver primary care where it matters most by conducting comprehensive home health risk assessments for Medicare Advantage members throughout Ocean County, NJ. This per diem opportunity offers immense flexibility as you help members in locations like Lakewood, Toms River, and Brick stay informed and empowered about their health directly from the comfort of their homes.

Schedule:

Key Responsibilities:

• Complete thorough annual and post-discharge home health risk assessments during 45-minute visits
• Evaluate a members current and past medical history, address chronic conditions, and review all medications
• Provide education to patients about their health status and assist in understanding treatment and medication regimens
• Identify gaps in care and offer actionable recommendations to improve overall patient outcomes
• Focus solely on assessmentsno requirements for creating treatment plans or prescribing medications

Qualifications:

• Active Nurse Practitioner license in New Jersey
• Board certified by AANP or ANCC
• CPR/BLS certification required and current
• MSN from an accredited school of nursing
• At least 6 months of NP experience is preferred
