Enjoy the autonomy and flexibility of a per diem Nurse Practitioner position focused on home health risk assessments for Medicare Advantage members throughout Ocean County, NJincluding Lakewood, Toms River, and Brick. In this community-centered role, you will visit members in their homes, with each evaluation taking approximately 45 minutes and ranging from annual health assessments to post-discharge visits.
Schedule:
- Flexible shifts available Monday through Sunday
- Design your own work schedule
- Choose a minimum weekly or bi-weekly hour commitment
Key Responsibilities:
- Conduct comprehensive annual home health risk assessments for Medicare Advantage plan members
- Review prior and current medical history, including medical conditions and medication use
- Educate members about their health conditions and prescribed medications during visits
- Identify and document gaps in care, providing clear recommendations to support quality outcomes
- No prescribing or creation of treatment plans requiredfocus is purely on thorough assessment and patient education
Qualifications:
- Unrestricted Nurse Practitioner license in New Jersey
- Current BLS (CPR) certification
- Board Certified through AANP or ANCC
- MSN from an accredited institution
- At least 6 months of recent experience as a Nurse Practitioner preferred
