Deliver impactful care and bring your expertise to the forefront by conducting in-home health risk assessments for Medicare Advantage plan members throughout Ocean County, NJ. This per diem opportunity is a fit for professionals looking for schedule flexibility and the chance to make a difference in the community through focused, patient-centered visits. Each assessment lasts about 45 minutes and may include annual wellness checks or post-discharge follow-ups.
Schedule:
- Flexible shifts available Monday through Sunday
- Create your own schedule to fit your lifestyle
- Meet minimum hourly commitments on a weekly or bi-weekly basis
Key Responsibilities:
- Conduct comprehensive annual home health risk assessments for Medicare Advantage members
- Review patients prior and current medical history, including all conditions and medications
- Offer personalized education regarding health status and medication management
- Identify and report any unresolved care gaps, providing recommendations for follow-up
- Note: No requirements for creating treatment plans or prescribing medications
Qualifications:
- Active Nurse Practitioner license in New Jersey
- Current CPR (BLS) certification
- Board Certification through AANP or ANCC
- Minimum of 6 months recent Nurse Practitioner experience preferred
- MSN from an accredited nursing program
