Deliver high-quality patient care directly in the community by performing home health risk assessments for Medicare Advantage members in Campbell County, TN. This per diem opportunity offers you the autonomy to create your own schedule and work flexible shifts across the week, including both weekdays and weekends. Visits are scheduled in local areas, ensuring you provide vital clinical assessments and education right where it's needed most.
Schedule:
- Flexible shifts available Monday through Sunday
- Ability to create your own schedule
- Minimum weekly or bi-weekly hourly commitment required
Key Responsibilities:
- Perform comprehensive annual or post-discharge home health risk assessments (average 45 minutes per visit)
- Review and evaluate prior and current medical history, medical conditions, and medications
- Educate patients on their health conditions and medication management
- Identify gaps in care and recommend solutions to enhance patient well-being
- No requirement to prescribe medications or develop treatment plans
Qualifications:
- Current and valid Nurse Practitioner license in TN
- Active CPR certification (BLS)
- Board Certification through AANP or ANCC
- MSN degree from an accredited nursing school
- 6 months of recent experience as a Nurse Practitioner is preferred
