This per diem opportunity offers Nurse Practitioners the flexibility to deliver essential home health risk assessments across Allegheny County. Assignments may take you to locations such as Pittsburgh, Penn Hills, or Mount Lebanon within the county, providing a dynamic work environment and opportunities to serve a diverse patient population.

Schedule:

• Flexible shifts, Monday through Sunday
• Create your own work schedule
• Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

• Conduct comprehensive home health risk assessments for Medicare Advantage plan members
• Review patients medical history, current medical conditions, and medication usage
• Educate patients on managing health conditions and understanding their medications
• Identify and report care gaps, recommending solutions
• No prescribing or treatment plan development is required

Qualifications:

• Active Nurse Practitioner License in Pennsylvania
• Board Certification through AANP or ANCC
• Current CPR (BLS) certification
• MSN degree from an accredited nursing program
• At least 6 months current Nurse Practitioner experience preferred

This role offers competitive compensation of $80-$90 per evaluation, plus a mileage stipend and paid administrative time.
