Step into an impactful per diem opportunity as a Nurse Practitioner, conducting home health risk assessments for Medicare Advantage plan members across Calcasieu County, Louisianaincluding the communities of Lake Charles, Sulphur, and DeQuincy. Utilize your advanced clinical skills to support patient well-being while enjoying control over your schedule.

Schedule/Hours:

  • Flexible shifts available Monday through Sunday
  • Create your own schedule to fit your lifestyle
  • Commit to a minimum number of hours weekly or bi-weekly

Key Responsibilities:

  • Perform comprehensive annual and post-discharge home health risk assessments for Medicare Advantage members
  • Review and document prior and current medical history, conditions, and medication usage
  • Offer patient education regarding health conditions and prescribed medications
  • Identify care gaps, providing recommendations to help address these needs
  • Maintain focus on assessmentsno treatment planning or prescribing required

Qualifications:

  • Active Nurse Practitioner license in Louisiana
  • Board Certification via AANP or ANCC
  • Current CPR certification (BLS)
  • Minimum of 6 months experience as an NP is preferred
  • MSN degree from an accredited institution
