Schedule:

• Flexible shifts between Monday and Sunday

• Choose minimum weekly or bi-weekly hourly commitments tailored to your availability

Key Responsibilities:

• Complete annual home health risk assessments for Medicare Advantage members
• Review, evaluate, and document medical history, current conditions, and medication usage
• Educate patients regarding their diagnoses, health status, and prescribed medications
• Identify and communicate gaps in care, offering recommendations for optimal health outcomes
• No requirement to prescribe medications or develop ongoing treatment plans

Qualifications:

• Active Nurse Practitioner license in Idaho
• Board Certification through AANP or ANCC
• MSN degree from an accredited nursing program
• Current CPR certification (BLS)
• Minimum of 6 months of current NP experience preferred

Compensation for this per diem role is structured at $80$90 per evaluation, with additional mileage stipends and administrative time included to recognize your commitment and travel.
