As a Nurse Practitioner performing home health risk assessments, youll be making a significant difference for Medicare Advantage plan members throughout Putnam County, including communities such as Palatka, Interlachen, and Crescent City. This per diem position is ideal for clinicians seeking autonomy, flexibility, and meaningful, one-on-one patient interactions.
Schedule:
- Flexible shifts available Monday through Sunday
- Set your own schedule to fit your life
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Conduct comprehensive annual home health risk assessments for Medicare Advantage members in their homes
- Review and evaluate prior and current medical history, chronic and acute conditions, and medication histories
- Educate patients on their medical conditions and proper medication management
- Identify gaps in care and provide actionable recommendations to improve patient outcomes
- Focus on assessment; no treatment planning or prescribing required
Qualifications:
- Active Nurse Practitioner license in Florida
- Current BLS/CPR certification
- Board Certification through AANP or ANCC
- MSN degree from an accredited nursing program
- Minimum of 6 months nurse practitioner experience preferred
