Job Description

Seeking an experienced Nurse Practitioner to conduct home health risk assessments for Medicare Advantage plan members across Victoria County, TX. In this per diem opportunity, you will play a vital part in assessing patient needs and ensuring continuity of care within a flexible and autonomous work environment.

Schedule/Hours:

  • Flexible shifts between Monday and Sunday
  • Design your own schedule
  • Minimum weekly or bi-weekly hourly commitment

Responsibilities:

  • Perform comprehensive home health risk assessments, averaging 45 minutes, for Medicare Advantage members
  • Review and evaluate prior and current medical history, chronic conditions, and medication regimen
  • Deliver clear education to patients on their health conditions and prescribed medications
  • Identify care gaps and offer actionable recommendations to improve patient outcomes
  • Assignments may include annual health checkups or post-discharge visits
  • No requirement for treatment plan development or prescription writing

Qualifications:

  • Valid Nurse Practitioner license in Texas
  • Board certification (AANP or ANCC)
  • Graduate of an accredited MSN program
  • Current CPR certification (BLS)
  • At least 6 months of recent NP experience preferred
  • Strong assessment, organizational, and communication skills
  • Comfortable working independently and traveling to various locations within Victoria County
