Deliver impactful care in the community as a Nurse Practitioner providing home health risk assessments within Washington County, MS. This per diem position offers a unique opportunity to work directly with Medicare Advantage plan members in their homesserving areas such as Greenville, Leland, and Hollandale. Schedule:

• Flexible shifts, MondaySunday
• Create your own schedule
• Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

• Perform annual health risk assessments for Medicare Advantage plan members in home settings
• Review and evaluate prior/current medical histories, conditions, and medication regimens
• Educate members about their health conditions and prescribed medications
• Identify gaps in care and suggest actionable recommendations
• Complete thorough assessmentsno treatment planning or prescribing required
• Average assessment duration: approximately 45 minutes

Qualifications:

• Active Nurse Practitioner license in MS
• Current CPR certification (BLS)
• Board Certified through AANP or ANCC
• Minimum of 6 months recent experience as an NP preferred
• Master of Science in Nursing (MSN) from an accredited program

Compensation and Benefits: Earn $80$90 for each evaluation, plus mileage stipend and compensated administrative time.
