Job Description

This per diem opportunity provides exceptional scheduling flexibilitytailor your hours with minimum weekly or bi-weekly commitments and choose from shifts available Monday through Sunday.

Schedule:

Key Responsibilities:

  • Perform comprehensive annual home health risk assessments in members homes
  • Review and evaluate prior and current medical histories, medication use, and medical conditions
  • Deliver clear education regarding medical conditions and medications to patients
  • Identify and address gaps in care, recommending practical solutions for patient wellness
  • Focus solely on assessmentno requirement to create treatment plans or prescribe medications

Qualifications:

  • Active Nurse Practitioner License in Indiana
  • Current BLS/CPR certification
  • Board Certified via AANP or ANCC
  • Masters degree in Nursing from an accredited institution
  • Minimum of 6 months of current Nurse Practitioner experience preferred

This role offers compensation of $80-$90 for each evaluation performed, along with mileage reimbursement and compensation for administrative time.
