Advance your career as a Nurse Practitioner in home health by conducting thorough risk assessments for Medicare Advantage plan members in Hendricks County, IN, including Plainfield, Brownsburg, and Avon. This per diem opportunity provides exceptional scheduling flexibilitytailor your hours with minimum weekly or bi-weekly commitments and choose from shifts available Monday through Sunday.
Schedule:
- Flexible shifts between Monday and Sunday

Key Responsibilities:
- Perform comprehensive annual home health risk assessments in members homes
- Review and evaluate prior and current medical histories, medication use, and medical conditions
- Deliver clear education regarding medical conditions and medications to patients
- Identify and address gaps in care, recommending practical solutions for patient wellness
- Focus solely on assessmentno requirement to create treatment plans or prescribe medications
Qualifications:
- Active Nurse Practitioner License in Indiana
- Current BLS/CPR certification
- Board Certified via AANP or ANCC
- Masters degree in Nursing from an accredited institution
- Minimum of 6 months of current Nurse Practitioner experience preferred
This role offers compensation of $80-$90 for each evaluation performed, along with mileage reimbursement and compensation for administrative time.
