Bring your advanced clinical skills to a dynamic per diem opportunity performing home health risk assessments for Medicare Advantage members throughout Hancock County, IN, including communities such as Greenfield, McCordsville, and Fortville. As a Nurse Practitioner in this role, youll play a critical part in supporting the health and wellness of patients in their homes while enjoying the autonomy and flexibility to shape your own work schedule.
Schedule:
- Flexible shifts available Monday through Sunday
- Set your own workdays and hours
- Choose your weekly or bi-weekly commitment based on your availability
Key Responsibilities:
- Perform comprehensive annual home health risk assessments for Medicare Advantage members
- Review and evaluate patients prior/current medical history, chronic conditions, and medication regimens
- Deliver patient education regarding health conditions and prescribed medications
- Identify care gaps and recommend interventions to address unmet health needs
- No need to develop ongoing treatment plans or prescribe medications
Qualifications:
- Active Nurse Practitioner license in Indiana
- Board certification through AANP or ANCC
- Master of Science in Nursing (MSN) from an accredited institution
- Current CPR certification (BLS)
- At least 6 months of nurse practitioner experience preferred
As part of this role, each in-home evaluation takes approximately 45 minutes and may involve various assessment types, including annual and post-discharge visits. Youll receive competitive pay of $80-$90 for each evaluation, as well as mileage and administrative stipends.
