Advance your career making a meaningful difference in patient care as a Nurse Practitioner delivering home health assessments. This per diem opportunity involves conducting in-home risk assessments for Medicare Advantage members throughout Cass County, Missouri, including areas such as Harrisonville, Raymore, and Cleveland. Each assessment typically takes approximately 45 minutes, and may include annual health evaluations or post-discharge visits. The role focuses on comprehensive evaluations, patient education, and care gap identificationwithout treatment planning or prescribing responsibilities.

Schedule:

• Flexible shifts available Monday through Sunday
• Freedom to set your own schedule
• Ability to select minimum weekly or bi-weekly hourly commitments

Responsibilities:

• Perform annual home health risk assessments for Medicare Advantage members in designated counties
• Evaluate current and prior medical history, chronic conditions, and medication regimens
• Provide clear education about medical diagnoses and prescribed medications
• Identify and document gaps in care; offer actionable recommendations to address them
• Maintain thorough, accurate clinical documentation

Qualifications:

• Active Nurse Practitioner License in Missouri
• Board Certified through AANP or ANCC
• Current CPR (BLS) certification
• Completion of MSN degree from an accredited nursing school
• At least 6 months of current NP experience (preferred)
• Strong assessment, communication, and organizational skills

You will receive $80-$90 per evaluation, as well as a mileage stipend and compensation for administrative time.
