Job Description
Take part in delivering high-quality care as a Nurse Practitioner specializing in home health risk assessments for Medicare Advantage plan members. Working throughout Idaho Falls and surrounding areas in Bonneville County, you will visit patients in their homes to perform comprehensive, 45-minute health assessments. These evaluations help improve care management and patient outcomes, and may include annual health assessments or post-discharge visits.
Schedule:
- Flexible shifts available Monday through Sunday
- Choose your own schedule
- Weekly or bi-weekly hourly commitment required
Key Responsibilities:
- Perform thorough annual home health risk assessments for Medicare Advantage members
- Review and evaluate patient medical history, current conditions, and medications
- Educate patients regarding their health status and current treatments
- Identify care gaps and provide actionable recommendations to support better outcomes
- No treatment planning or prescribing required
Qualifications:
- Active Nurse Practitioner license in Idaho
- Current CPR (BLS) certification
- Board Certification through AANP or ANCC
- Minimum six months of NP experience preferred
- Master of Science in Nursing (MSN) from an accredited program
Compensation includes $80$90 per evaluation, plus a mileage stipend for travel and remuneration for administrative time.
