Job Description

Step into a vital clinical role delivering in-home health risk assessments across Custer County, OK. This per diem opportunity empowers Nurse Practitioners to provide compassionate, individualized care with flexible scheduling options designed to fit your lifestyle. Youll visit Medicare Advantage plan members in their homes, covering areas such as Weatherford, Arapahoe, Clinton, Thomas, and more within the county.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Create your own schedule to match your availability
  • Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

  • Conduct comprehensive annual health risk assessments during home visits for Medicare Advantage members
  • Gather and evaluate detailed patient medical history, current health conditions, and medication regimens
  • Educate patients on the management of medical conditions and prescribed medications
  • Identify gaps in patient care and offer actionable recommendations to close those gaps
  • Documentation and reporting of findings according to established guidelines
  • No requirements for creating treatment plans or prescribing medications

Qualifications:

  • Active Nurse Practitioner license in the state of Oklahoma
  • Board certification from AANP or ANCC required
  • Current CPR (BLS) certification
  • Minimum of 6 months current Nurse Practitioner experience preferred
  • MSN from an accredited nursing program
