Job Description

Make a meaningful impact by conducting comprehensive home health risk assessments for Medicare Advantage plan members across Wood County, including Parkersburg, Vienna, and Williamstown. This per diem opportunity offers the flexibility to design your own work schedule and take on shifts that suit your lifestyle.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Autonomy to create your own schedule
  • Set your preferred minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

  • Perform annual and post-discharge home health risk assessments for Medicare Advantage members
  • Review and evaluate medical history, current conditions, and medication usage
  • Educate patients on chronic conditions, medications, and preventive health strategies
  • Identify care gaps and provide recommendations to address them
  • Record findings thoroughly; no responsibility for prescribing or developing treatment plans

Qualifications:

  • Active Nurse Practitioner license in West Virginia
  • Board Certification via AANP or ANCC
  • Current CPR certification (BLS)
  • MSN from an accredited nursing program
  • At least 6 months of nurse practitioner experience preferred

This position compensates $80-$90 per evaluation, and includes a mileage stipend as well as remuneration for administrative time.
