Step into a meaningful per diem role where your expertise supports Medicare Advantage members through comprehensive home health risk assessments. In this home health setting, youll be completing in-person visits across Huron County, including areas like Norwalk, Willard, and Bellevue. Assessments average 45 minutes each and may vary from annual health visits to post-discharge check-ins, providing valuable insights to enhance patient care.

Schedule:

• Flexible shifts available Monday through Sunday
• Build your own schedule
• Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

• Perform in-home annual risk assessments for plan members
• Review and document prior/current medical history and medication use
• Evaluate health status, identifying gaps in care and providing recommendations
• Educate patients on their medical conditions and prescribed treatments
• No need to write treatment plans or prescribe medications

Qualifications:

• Active Nurse Practitioner license in Ohio
• Board certification from AANP or ANCC
• Current CPR (BLS) certification
• At least 6 months of experience as an NP preferred
• Master of Science in Nursing degree from an accredited program

This position offers compensation of $80$90 per evaluation, along with a mileage stipend and payment for administrative time.
