Take on a dynamic per diem opportunity as an experienced Nurse Practitioner conducting in-home health risk assessments across Grant County, NM and surrounding communities. Schedule:
- Flexible shifts available Monday through Sunday
- Self-scheduling and autonomy to create your work calendar
- Commitment can be structured weekly or bi-weekly based on preference
Key Responsibilities:
- Perform annual home health risk assessments for Medicare Advantage plan members in varied locations within the county (including Hurley, Bayard-Santa Rita, Santa Clara, and Tyrone)
- Review and analyze patients' prior and current medical histories, health conditions, and medications
- Educate patients regarding their medical conditions and prescribed medications
- Identify care gaps and recommend strategies to address unresolved health needs
- Complete each assessment within approximately 45 minutes, focusing on evaluation rather than ongoing treatment or prescribing
Qualifications:
- Active Nurse Practitioner license in the state of NM
- Board certified through AANP or ANCC
- Current CPR (BLS) certification
- Minimum of 6 months recent NP experience preferred
- Masters degree (MSN) from an accredited nursing program
This role offers competitive per-evaluation earnings between $80-$90, along with a mileage stipend and compensation for administrative time.
