Job Description

Deliver exceptional patient care with per diem flexibility as a Nurse Practitioner specializing in home health risk assessments. In this dynamic role, you will visit Medicare Advantage plan members throughout Carroll County, ARincluding Berryville, Green Forest, and Holiday Islandto conduct thorough annual or post-discharge assessments. Each home visit typically lasts around 45 minutes, providing a meaningful opportunity to impact patient well-being across a diverse population.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Self-scheduling with the opportunity to create your own workdays
  • Minimum weekly or bi-weekly hourly commitment for continuity

Responsibilities:

  • Perform comprehensive home health risk assessments for Medicare Advantage members
  • Review prior and current medical histories, evaluate ongoing conditions, and verify medication lists
  • Educate patients about their conditions and medications to promote understanding and self-management
  • Identify care gaps and provide actionable recommendations to support better health outcomes
  • Focus assessments on documentation and patient educationno duties involving treatment plan creation or medication prescribing

Desired Qualifications:

  • Active Nurse Practitioner license in the state of Arkansas
  • Board certification as an NP through AANP or ANCC
  • Current Basic Life Support (BLS/CPR) certification
  • MSN degree conferred by an accredited nursing program
  • At least 6 months of recent experience as a Nurse Practitioner preferred
