Enjoy a flexible per diem role as a Nurse Practitioner focused on comprehensive home health risk assessments for Medicare Advantage plan members. This position serves communities within Cabarrus County, including Concord, Kannapolis, and Locust. As a valued healthcare provider, youll bring high-quality assessment and educational skills directly to plan members homes, supporting preventive care and overall wellness.
Schedule:
- Flexible shifts available Monday through Sunday
- Design your own work schedule
- Choose weekly or bi-weekly hourly commitments to suit your availability
Requirements:
- Active Nurse Practitioner license in North Carolina
- Current CPR certification (BLS)
- Board Certification through AANP or ANCC
- Minimum of 6 months recent Nurse Practitioner experience preferred
- MSN degree from an accredited nursing program
Key Responsibilities:
- Perform thorough home health risk assessments for Medicare Advantage members during each appointment
- Assess prior and current medical history, general health conditions, and review medication usage
- Educate patients on medical conditions and medications, promoting patient understanding and compliance
- Detect any gaps in care, offering evidence-based recommendations to address those needs
- No requirement for developing treatment plans or writing prescriptions
What Youll Receive: Compensation for this role is $80$90 per evaluation, plus a mileage stipend and administrative time allowance.
