In this rewarding per diem opportunity, youll enjoy the flexibility to tailor your schedule and work with Medicare Advantage plan members across the region, including Jackson, Medon, and Three Way.
Schedule:
- Flexible shifts available Monday through Sunday
- Self-scheduling for optimal work-life balance
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Perform comprehensive annual home health risk assessments for Medicare Advantage health plan members
- Review and evaluate current and prior medical history, including conditions and medications
- Educate patients on their medical conditions and prescribed treatments
- Identify care gaps and offer recommendations to ensure continuity of care
- Visits may include annual health assessments or post-discharge evaluations; no prescription or treatment planning required
Qualifications:
- Active Nurse Practitioner license in Tennessee
- Board Certification from AANP or ANCC
- Current BLS CPR certification
- MSN degree from an accredited nursing program
- At least 6 months of recent Nurse Practitioner experience is preferred
