In this rewarding per diem opportunity, youll enjoy the flexibility to tailor your schedule and work with Medicare Advantage plan members across the region, including Jackson, Medon, and Three Way.

Schedule:

• Flexible shifts available Monday through Sunday
• Self-scheduling for optimal work-life balance
• Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

• Perform comprehensive annual home health risk assessments for Medicare Advantage health plan members
• Review and evaluate current and prior medical history, including conditions and medications
• Educate patients on their medical conditions and prescribed treatments
• Identify care gaps and offer recommendations to ensure continuity of care
• Visits may include annual health assessments or post-discharge evaluations; no prescription or treatment planning required

Qualifications:

• Active Nurse Practitioner license in Tennessee
• Board Certification from AANP or ANCC
• Current BLS CPR certification
• MSN degree from an accredited nursing program
• At least 6 months of recent Nurse Practitioner experience is preferred
