Schedule:

Key Responsibilities:

  • Perform home health risk assessments for Medicare Advantage plan members in areas such as Humboldt, Milan, and Trenton
  • Assess prior and current medical history, conditions, and medication use
  • Educate members on disease management, medication adherence, and preventive care
  • Identify care gaps and suggest solutions to improve members overall health
  • No requirements for prescribing medication or developing treatment plans

Qualifications:

  • Active Nurse Practitioner license in Tennessee
  • MSN degree from an accredited nursing school
  • Board Certification by AANP or ANCC
  • Valid CPR (BLS) certification
  • At least 6 months of recent experience as an NP preferred
