Schedule:
- Flexible shifts available seven days a week
- Create a personalized schedule that fits your availability
- Minimum commitments available weekly or bi-weekly
Key Responsibilities:
- Perform home health risk assessments for Medicare Advantage plan members in areas such as Humboldt, Milan, and Trenton
- Assess prior and current medical history, conditions, and medication use
- Educate members on disease management, medication adherence, and preventive care
- Identify care gaps and suggest solutions to improve members overall health
- No requirements for prescribing medication or developing treatment plans
Qualifications:
- Active Nurse Practitioner license in Tennessee
- MSN degree from an accredited nursing school
- Board Certification by AANP or ANCC
- Valid CPR (BLS) certification
- At least 6 months of recent experience as an NP preferred
