Job Description

Youll visit patients in their homes within St. Francis County, delivering compassionate, thorough assessments for Medicare Advantage plan members in diverse areas, including Forrest City, Hughes, and Madison.

Schedule:

  • Flexible shifts available seven days a week
  • Create your own schedule to fit your availability
  • Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

  • Conduct annual and post-discharge home health risk assessments for Medicare Advantage plan members.
  • Review and document medical, surgical, and medication histories.
  • Educate patients on health conditions, preventive guidelines, and safe medication usage.
  • Identify unmet health needs or care gaps and recommend next steps.
  • No responsibility for developing treatment plans or prescribing medications.

Qualifications:

  • Active Nurse Practitioner license in Arkansas.
  • Board Certification from AANP or ANCC.
  • Current Basic Life Support (BLS) certification.
  • MSN from an accredited nursing program.
  • At least 6 months experience as an NP is preferred.
  • Reliable transportation for in-home visits throughout St. Francis County.

You will receive $80-$90 for each evaluation completed, as well as stipends covering mileage and administrative tasks associated with your visits.
