This role centers on conducting home health risk assessments for Medicare Advantage plan members within Kent County, MIcovering areas like Grand Rapids, Wyoming, and Kentwood. Schedule:
- Flexible shifts Monday through Sunday
- Choose your preferred working days and hours
- Minimum weekly or bi-weekly hour requirement
Key Responsibilities:
- Complete annual home health risk assessments for Medicare Advantage members
- Review and assess prior/current medical history, existing medical conditions, and medication histories
- Provide thorough education on health conditions and medications to patients during visits
- Identify gaps in care and recommend next steps to improve patient outcomes
- Focus solely on assessmentsno treatment plan development or medication prescription involved
Qualifications:
- Active Nurse Practitioner license in Michigan
- Current Basic Life Support (BLS/CPR) certification
- Board certified via AANP or ANCC
- At least 6 months of recent NP experience preferred
- Masters degree in Nursing (MSN) from an accredited institution
