Job Description

Make a meaningful impact by providing high-quality care and assessments in a per diem home health role. This opportunity offers Nurse Practitioners the flexibility to design their ideal work schedule while completing comprehensive risk assessments for Medicare Advantage plan members throughout Greene County, NYincluding Catskill, Cozsackie, Ashland, and Ravena.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Set your own schedule to fit your lifestyle
  • Commit to a minimum weekly or bi-weekly number of hours

Key Responsibilities:

  • Conduct annual home health risk assessments for Medicare Advantage plan members in their homes
  • Review and evaluate prior and current medical and medication history
  • Educate patients on chronic conditions, diagnoses, and medication safety
  • Identify any gaps in care, providing recommendations for improvement
  • Document all findings and recommendations during each approximately 45-minute assessment
  • Note: This role does not involve prescribing medications or creating treatment plans

Qualifications:

  • Active Nurse Practitioner license in the state of NY
  • Master of Science in Nursing (MSN) from an accredited institution
  • Board Certification through AANP or ANCC
  • Current CPR/BLS certification
  • At least 6 months of current NP experience is preferred

This flexible, per diem opportunity offers $80$90 per evaluation, plus a mileage stipend and compensation for administrative tasks.
