A unique opportunity awaits experienced Nurse Practitioners passionate about home health care. This per diem position involves conducting home health risk assessments for Medicare Advantage plan members throughout Burke County, NC. Visits may include annual health assessments or post-discharge evaluations, each lasting approximately 45 minutes. Practitioners may be scheduled in various communities across Burke Countyincluding Morganton, Hickory, and Valdesebased on need.

Responsibilities:
- Perform comprehensive annual home health risk assessments in patient residences
- Review and assess medical histories, chronic conditions, and medication use
- Educate patients regarding their diagnoses, care plans, and prescriptions
- Identify any gaps in care, providing actionable recommendations to resolve them
- No requirement for treatment plan creation or medication prescribing
Qualifications:
- Active Nurse Practitioner license in North Carolina
- Certified in Basic Life Support (CPR/BLS)
- Board certified through AANP or ANCC
- At least 6 months of recent experience as a Nurse Practitioner preferred
- Master of Science in Nursing (MSN) from an accredited institution
