Schedule:

• Choose your own schedule with flexible shifts, Monday through Sunday
• Minimum weekly or bi-weekly hourly commitment based on your availability

Key Responsibilities:

• Complete thorough annual home health risk assessments (approx. 45 minutes each) for Medicare Advantage members in their homes
• Review, evaluate, and document patients prior and current medical history, ongoing conditions, and medication history
• Provide clear education to patients on their conditions and medications
• Identify gaps in care, offering focused recommendations to improve patient outcomes
• Visits may include annual health assessments and post-discharge check-ins
• Please note: No prescribing or treatment planning is required

Qualifications:

• Active Nurse Practitioner License in New York
• Board Certification through AANP or ANCC
• Master of Science in Nursing (MSN) from an accredited institution
• Current CPR (BLS) certification
• At least six months of current NP experience preferred

You will receive payment of $80$90 per evaluation, with additional mileage stipend and compensation for administrative time, recognizing the value of your skills and commitment.
