Take on a pivotal role as a Nurse Practitioner delivering compassionate, high-quality home health care while enjoying flexible, per diem scheduling. In this position, you will perform home health risk assessments for Medicare Advantage plan members throughout Onslow County, supporting patient well-being and proactive care. Youll have the freedom to design your own schedule and make a meaningful impact on the health of individuals in diverse communities.

Schedule:

• Flexible shifts available seven days a week
• Design your own schedule to meet personal and professional needs
• Choose between minimum weekly or bi-weekly hourly commitments

Key Responsibilities:

• Perform annual home health risk assessments for Medicare Advantage members
• Review and evaluate patients medical history, current health conditions, and medication use
• Provide patient education regarding any chronic or acute health conditions, and review medication regimens
• Assess for gaps in care, documenting and recommending action where needed
• Visits typically last around 45 minutes; visits may be annual assessments or post-discharge follow-ups
• No treatment planning or medication prescribing required

Qualifications:

• Active Nurse Practitioner License in North Carolina
• Current BLS/CPR certification
• National board certification as an NP (AANP or ANCC)
• Minimum of six months NP experience preferred
• MSN degree from an accredited nursing program
