This per diem position is ideal for compassionate and independent Nurse Practitioners who value flexibility and want to work directly within the community. Youll conduct thorough health evaluations in patients homes, ensuring high-quality care and proactive support.
Schedule:
- Flexible shifts between Monday and Sunday
- Set your own schedule
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Perform annual home health risk assessments for Medicare Advantage plan members residing within Delta County
- Evaluate patients' prior and current medical history, ongoing medical conditions, and medication use
- Offer patient education regarding disease management and medication adherence
- Identify gaps in care and recommend actionable steps to close them
- Documentation of assessment findings without developing treatment plans or prescribing medications
Qualifications:
- Current Nurse Practitioner license in Colorado
- Board certified by AANP or ANCC
- MSN degree from an accredited school of nursing
- Active BLS (CPR) certification
- Minimum of six months of Nurse Practitioner experience preferred
