Expand your clinical expertise while making a meaningful impact through in-home patient evaluations. This per diem opportunity is ideal for a Nurse Practitioner with experience in home health, looking for a flexible schedule and autonomy in their work.

Schedule/Hours:

• Flexible shifts offered Monday through Sunday
• Self-scheduling: create your own work calendar
• Minimal weekly or bi-weekly hourly commitment required

Key Responsibilities:

• Perform comprehensive home health risk assessments for Medicare Advantage members in Alleghany County and surrounding areas, including Clifton Forge, Covington, and Iron Gate
• Conduct evaluations that last approximately 45 minutes per patient, which may include annual health or post-discharge assessments
• Review and assess prior/current medical history, medical conditions, and medication use
• Deliver patient education regarding health status and medication management
• Identify gaps in care and advise on next steps to resolve issuesno prescribing or treatment planning required

Qualifications:

• Active Nurse Practitioner license in Virginia
• Board Certification via AANP or ANCC
• Current, valid CPR certification (BLS)
• MSN from an accredited nursing program
• At least 6 months current NP experience preferred
