Advance your clinical expertise while supporting community health as a Nurse Practitioner specializing in home health risk assessments. This per diem opportunity involves conducting comprehensive evaluations for Medicare Advantage plan members in various locations throughout Calhoun County. Youll have the flexibility to design your own schedule, making it ideal for those balancing multiple commitments or seeking adaptable hours.
Schedule:
- Flexible shifts available Monday through Sunday
- Design your own work schedule
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Perform annual health risk assessments in the homes of Medicare Advantage plan members
- Assess previous and current medical history, review medication regimens, and evaluate existing health conditions
- Educate patients regarding their medical conditions and medications
- Identify gaps in care, making recommendations for appropriate follow-up or management
- Visits may range from annual assessments to post-discharge follow-ups
- No requirement to prescribe medications or develop treatment plans
Qualifications:
- Active Nurse Practitioner license in FL
- Board Certification through AANP or ANCC
- Current CPR (BLS) certification
- MSN degree from an accredited nursing program
- Minimum of six months of recent experience as a Nurse Practitioner preferred
As part of this role, youll receive $80$90 per completed evaluation, in addition to a mileage stipend and administrative time compensation. This structure supports your professional contributions and acknowledges the travel unique to in-home care.
