Deliver specialized care and make a meaningful impact as a Nurse Practitioner providing home health risk assessments for Medicare Advantage plan members across Burnet County, TXincluding Burnet and Marble Falls. This per diem opportunity allows you to work closely with patients in the comfort of their homes and help identify crucial health needs while enjoying a flexible work arrangement.

Schedule:

• Flexible shifts between Monday and Sunday
• Ability to create your own schedule
• Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

• Perform in-home health risk assessments for Medicare Advantage health plan members
• Review prior and current medical histories, medical conditions, and medication profiles
• Educate patients about their health conditions and prescribed medications
• Identify gaps in care and recommend strategies for resolution
• Each risk assessment visit averages about 45 minutes
• No responsibility for creating treatment plans or prescribing medications

Qualifications:

• Active Nurse Practitioner license in Texas
• Board Certification from AANP or ANCC required
• Current CPR certification (BLS)
• MSN degree from an accredited nursing school
• Minimum of 6 months current NP experience is preferred
• Excellent interpersonal and communication skills
