Schedule:

Responsibilities:
- Conduct comprehensive annual home health risk assessments for Medicare Advantage plan members
- Evaluate patients prior and current medical history, medical conditions, and medication regimens during in-home visits (approximately 45 minutes per assessment)
- Educate members about their health conditions and prescribed medications
- Identify gaps in care and provide actionable recommendations to address any deficiencies
- Perform visits without responsibility for developing treatment plans or prescribing medications
Qualifications:
- Active and unrestricted Nurse Practitioner license in New York State
- Current CPR certification (BLS)
- Board Certification through AANP or ANCC
- Preferred minimum of 6 months of recent Nurse Practitioner experience
- Master of Science in Nursing (MSN) from an accredited nursing school
