Responsibilities:

  • Conduct comprehensive annual home health risk assessments for Medicare Advantage plan members
  • Evaluate patients prior and current medical history, medical conditions, and medication regimens during in-home visits (approximately 45 minutes per assessment)
  • Educate members about their health conditions and prescribed medications
  • Identify gaps in care and provide actionable recommendations to address any deficiencies
  • Perform visits without responsibility for developing treatment plans or prescribing medications

Qualifications:

  • Active and unrestricted Nurse Practitioner license in New York State
  • Current CPR certification (BLS)
  • Board Certification through AANP or ANCC
  • Preferred minimum of 6 months of recent Nurse Practitioner experience
  • Master of Science in Nursing (MSN) from an accredited nursing school
