Job Description
Bring your expertise in home health to a flexible, impactful opportunity as a Nurse Practitioner conducting risk assessments for Medicare Advantage members in Tioga County. This per diem position provides autonomy to manage your own schedule, with shifts available any day of the week and visits scheduled in Waverly, Owego, Newark Valley, and surrounding areas.
Schedule:
- Flexible shifts available Monday through Sunday
- Design your own work schedule
- Commitment on a weekly or bi-weekly basis
Key Responsibilities:
- Perform comprehensive annual or post-discharge home health risk assessments (approximately 45 minutes each) for Medicare Advantage plan members
- Review, document, and evaluate patients current and prior medical history, active medical conditions, and medication regimens
- Offer clear and supportive education to patients regarding their health conditions and current therapies
- Identify gaps in care and recommend solutions to optimize health outcomes
- No need to create treatment plans or prescribe medications
Qualifications:
- Active Nurse Practitioner license in the state of New York
- Current Board Certification (AANP or ANCC)
- Valid CPR certification (BLS)
- MSN from an accredited nursing program
- Minimum of 6 months recent NP experience preferred
This role offers compensation of $80-$90 per completed evaluation, with mileage stipend and administrative time included.
