Step into a pivotal role as a Nurse Practitioner focused on home health risk assessments within Chemung County, NY, including Horseheads, Elmira Heights, and Wellsburg. This per diem opportunity gives you the flexibility to create your own schedule, working shifts that suit you between Monday and Sunday, with a minimum weekly or bi-weekly hourly commitment.

Responsibilities:
- Conduct comprehensive home health risk assessments for Medicare Advantage members
- Complete concise evaluations, with each visit typically taking about 45 minutes
- Review prior and current medical histories, health conditions, and medications
- Educate patients about their conditions and prescribed treatments
- Identify gaps in care, providing clear recommendations for follow-up
- No requirement to create treatment plans or prescribe medications; focus is on assessment, education, and care coordination
Qualifications:
- Active Nurse Practitioner license in NY
- Board Certification (AANP or ANCC)
- MSN from an accredited nursing school
- Current CPR (BLS) certification
- At least 6 months of recent Nurse Practitioner experience preferred
