Job Description

Provide essential care and impact patients lives as a Nurse Practitioner performing home health risk assessments for Medicare Advantage plan members throughout Chemung County. In this per diem position, you have the flexibility to create your own schedule, with shifts available any day of the week and a minimum hourly commitment to match your availability. Responsibilities center on conducting 45-minute risk assessments in patient homes across locations such as Horseheads, Elmira Heights, Wellsburg, and surrounding areas.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Choose your own schedule each week
  • Minimal required hours on a weekly or bi-weekly basis

Key Responsibilities:

  • Conduct annual home health risk assessments for Medicare Advantage members
  • Review patient medical, surgical, and medication history
  • Offer education regarding medical conditions and current medication regimens
  • Identify gaps in care, providing actionable recommendations to patients
  • Visits may include annual wellness assessments or post-discharge evaluations
  • No requirement to develop treatment plans or prescribe medications

Qualifications:

  • Active Nurse Practitioner license in New York State
  • Current CPR certification (BLS)
  • Board certification via AANP or ANCC
  • MSN from an accredited nursing program
  • At least 6 months of recent Nurse Practitioner experience preferred
