Make a real difference in patient care while enjoying the flexibility of per diem work. This opportunity involves conducting home health risk assessments for Medicare Advantage plan members throughout Steuben County. Locations may include Corning, Gang Mills, Bath, Hornell, and adjacent communities, depending on individual shift assignments.
Schedule:
- Flexible shifts available Monday through Sunday
- Create your own work schedule
- Commitment of minimum weekly or bi-weekly hours
Key Responsibilities:
- Complete comprehensive annual home health risk assessments for Medicare Advantage members
- Review medical history, current conditions, and medications
- Educate patients on health management and medication usage
- Identify gaps in care and provide actionable recommendations to promote patient health
- Visits are assessment-only; no prescribing or treatment plans required
- Each assessment averages 45 minutes
Qualifications:
- Licensed Nurse Practitioner in New York State
- Current BLS/CPR certification
- Board certification through AANP or ANCC
- MSN from an accredited nursing program
- Six months or more of NP experience preferred
