Job Description

Experienced Nurse Practitioners are needed for flexible per diem opportunities conducting home health risk assessments with Medicare Advantage members in Steuben County, NY. This role centers on performing thorough in-home evaluations, focusing on annual health assessments or post-discharge visits, each typically lasting about 45 minutes. You will have the opportunity to serve members residing in areas such as Bath, Corning, Gang Mills, and Hornell, with scheduling done per shift in any of these locations.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Self-schedulingset your own hours
  • Minimum commitment required weekly or bi-weekly

Key Responsibilities:

  • Perform comprehensive annual home health risk assessments for Medicare Advantage members
  • Review prior and current medical history, medical conditions, and medication records
  • Educate members on medical diagnoses, preventive care, and appropriate medication usage
  • Recognize gaps in care and offer recommendations to improve health outcomes
  • Note: No prescribing or treatment plan development necessary

Qualifications:

  • Active Nurse Practitioner license in New York State
  • Board Certification through AANP or ANCC
  • Current CPR (BLS) certification
  • MSN degree from an accredited nursing program
  • At least 6 months of current NP experience preferred
