Make a meaningful impact by providing compassionate care and critical assessments to patients in their own homes. As a Nurse Practitioner specializing in home health, you'll conduct comprehensive risk assessments for Medicare Advantage members throughout Nodaway County, including areas such as Maryville, Burlington Junction, and Hopkins. This per diem role balances flexibility and autonomy, allowing you to design your work schedule while performing essential healthcare evaluations.

Schedule:

• Flexible shifts available between Monday and Sunday
• Set your own schedule to match your availability
• Choose your preferred level of weekly or bi-weekly commitments

Key Responsibilities:

• Perform in-home annual health risk assessments for Medicare Advantage plan members
• Review and evaluate patients medical history, ongoing conditions, and medication use
• Educate patients on chronic conditions, medication management, and preventive health measures
• Detect gaps in care, providing on-the-spot recommendations to address them
• No responsibility for developing treatment plans or prescribing medication

Qualifications:

• Active Nurse Practitioner license in Missouri
• Board Certification through AANP or ANCC
• Current CPR certification (BLS)
• MSN degree from an accredited nursing school
• At least 6 months of recent Nurse Practitioner experience preferred
