In this per diem opportunity, you will utilize your advanced clinical skills to offer valuable assessments and guidance to Medicare Advantage members residing in Patterson, Putnam Valley, Philipstown, and nearby areas. Enjoy the benefit of building your own flexible schedule, choosing shifts between Monday and Sunday, with a minimum weekly or bi-weekly hourly commitment.

Key Responsibilities:

• Conduct comprehensive annual home health risk assessments for Medicare Advantage plan members
• Review previous and current medical history, evaluate conditions, and create a full medication profile
• Provide education on medical conditions and medications tailored to each member
• Identify and address care gaps, recommending actionable steps for follow-up
• Visits are typically 45 minutes eachincluding both annual and post-discharge assessments
• No creation of treatment plans or medication prescribing required

Qualifications:

• Active Nurse Practitioner license in NY
• Current CPR (BLS) certification
• Board Certified as an NP through AANP or ANCC
• MSN degree from an accredited nursing program
• At least 6 months of current NP experience preferred
