Job Description

Seeking a highly motivated Nurse Practitioner to conduct home health risk assessments in various locations throughout Dutchess County, NY, including Poughkeepsie, Beacon, Fishkill, Rhinebeck, and nearby areas. This per diem opportunity allows for great flexibility, making it an excellent fit for professionals who value autonomy and a varied work environment. You will perform independent risk assessments for Medicare Advantage plan members in their homes, focusing on health status, medication review, and education.

Schedule:

  • Flexible shifts available any day between Monday and Sunday
  • Create your own schedule to balance with other commitments
  • Minimal weekly or bi-weekly hourly requirementstailored to your availability

Key Responsibilities:

  • Conduct annual home health risk assessments within member residences, each lasting roughly 45 minutes
  • Review and document prior/current medical histories, conditions, and medications
  • Offer education to members on their conditions and the safe use of medications
  • Identify potential gaps in care and recommend appropriate next steps or referrals
  • Focus on assessment and educationno requirement to develop treatment plans or prescribe medications

Qualifications:

  • Active Nurse Practitioner license in NY State
  • Board Certification via AANP or ANCC
  • Current CPR (BLS) certification
  • Preferably 6 months of recent nurse practitioner experience
  • MSN from an accredited nursing program

In addition to a rewarding clinical experience, this position is compensated at $80$90 per evaluation, plus a mileage stipend and administrative time.
