Job Description

As a Nurse Practitioner specializing in Home Health, you will conduct comprehensive risk assessments for Medicare Advantage plan members residing in various areas within Cumberland County, including Hope Mills, Spring Lake, Eastover, and surrounding communities. These assessments, typically taking about 45 minutes each, may cover annual health reviews or post-discharge visitsoffering vital insight into member well-being and continuity of care.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Self-scheduling allows you to select your preferred hours
  • Minimum weekly or bi-weekly hourly commitment required

Responsibilities:

  • Conduct in-home risk assessments, focusing on overall health, medical conditions, and medication compliance
  • Review prior and current medical history to ensure accurate reporting and patient understanding
  • Educate members on their medical conditions and the importance of medication adherence
  • Identify care gaps and provide actionable recommendations, fostering better health outcomes
  • No treatment plan development or medication prescribing involved, ensuring a focus on risk evaluation and education

Qualifications:

  • Active Nurse Practitioner license in North Carolina
  • Board Certification via AANP or ANCC
  • Current BLS/CPR certification
  • MSN degree from an accredited nursing program
  • Minimum of 6 months' recent Nurse Practitioner experience preferred
