Job Description

A rewarding per diem opportunity is available for experienced Nurse Practitioners who are passionate about delivering high-quality home health care. This position focuses on conducting comprehensive home health risk assessments for Medicare Advantage plan members throughout the various communities of Cumberland County. With flexible scheduling and the ability to design your own shifts, this role offers an excellent balance for professionals seeking autonomy and impact in their work.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Set your own schedule
  • Minimum commitment required on a weekly or bi-weekly basis

Key Responsibilities:

  • Perform annual risk assessments for individuals in their homes to support a Medicare Advantage health plan.
  • Thoroughly evaluate medical history, current conditions, and medication usage.
  • Educate members about their health conditions and treatments.
  • Pinpoint gaps in care and offer recommendations for improvement.
  • Focus on assessmentsthere is no need for treatment planning or prescription writing.
  • Each risk assessment typically takes about 45 minutes, with some visits being annual health evaluations or post-discharge follow-ups.

Qualifications:

  • Active Nurse Practitioner license in North Carolina.
  • Board Certification via AANP or ANCC.
  • Graduate of an accredited MSN program.
  • Current BLS (CPR) certification.
  • At least six months of experience as a Nurse Practitioner is preferred.

You will receive $80$90 per completed evaluation, plus a mileage stipend and compensation for administrative tasks involved with your visits.
